Student Registration

The Merrimack School District is now utilizing an online student registration system for all new and returning (currently enrolled) students.  This system replaces the paper forms sent home at the beginning of each school year. You will use the PowerSchool Parent Portal to access the registration system.

New Student Registration

Step 1

Download and complete the New Student Information Entry Form

Step 2

Return the New Student Information Entry Form with the following information to your neighborhood school:

  • Child's original birth certificate with raised seal (a copy will be made for records and original returned immediately)
  • Immunization record
  • Copy of most recent medical examination (within one year of enrollment year)
  • Proof of residency (utility bill or lease agreement and/or mortgage agreement)
  • If applicable, legal papers

To be eligible for kindergarten registration, a child must be five years old by September 30.  To be eligible for first grade registration, a child must be six years old by September 30.

All students registering for public school for the first time are required by state law (RSA 200:32) to have a physical examination by a licensed physician. This exam must take place within one year prior to the first day of school.

In accordance with RSA 200:38 and 141-C, the child is also required to be immunized for the following before entering school:  measles, mumps, rubella, diphtheria, pertussis and tetanus, oral and/or injected polio, varicella, and Hepatitis B vaccine.  The month, day and year are required for all immunizations.  For any changes on fall requirements, please visit the State of NH website at:
http://www.education.nh.gov/instruction/school_health/faq_immunizations.htm

No child will be allowed to enter school in the fall unless she/he has met all the above requirements.  Parents are urged to complete the physical examination and immunizations as soon as possible, as area doctors often have difficulty scheduling appointments late in the summer.  Please have your child’s medical record completed and returned to school by August 1.

The Nashua Public Health Department has immunization clinics for young children every week.  Immunizations are available for a small fee.  The center is located at 18 Mulberry Street, Nashua, NH.  Please call 589-4560 for the clinic schedule and directions.
