     If your child will need medications of any kind for the 2025-2026 school year at MMS, please follow the instructions below to be sure you have everything you need to submit to me in order for them to be able to take medications at our school.

    If the medication is a prescription medication, please print and fill out the form below (CLICK green link provided below ).   ONLY EPI-PENS, INSULIN AND/OR INHALERS ARE ALLOWED TO BE HAND CARRIED AND SELF ADMINISTERED AND AUTHORIZATON TO DO THIS MUST BE COMPLETED BY PHYSICIAN AND PARENT on the 2nd and 3rd sections of the Prescription Authorization Form.     All medications must be brought in BY A PARENT OR ADULT DESIGNEE and be in the original container or prescription bottle.    If you have any questions or concerns about this, please call me!!
     
    If you would like OVER THE COUNTER (OTC) medications available to your student during the school year, please fill out the first page of the form below...and initial the meds I either carry as stock and any others you will be providing, AND sign page #2.   Please do not initial every med on the form unless you intend on supplying all of these medications.  The only stock medications that I carry in my office are Advil, Extra Strength Tylenol and Tums.   If you want these medications available to your son/daughter, you must initial the medications you want available to them and sign the Over the Counter Administration form.  If your child is not able to swallow tablets of Tylenol or Advil, you must supply chewables or liquid form of these medications as I do not carry stock of them.  If you have another OTC medication that you would like to have available to your child that I do not carry stock of, you must supply it to me in its original container, making sure the supply is not expired!
    **If your child takes more than a very occasional amount of any of my stock medications, please send in their own supply!! 
     
     ** STUDENTS ARE NOT ALLOWED TO CARRY ANY OVER THE COUNTER or PRESCRIPTION (NON-EMERGENT) MEDICATIONS ON THEM**
     
    **IF THEY NEED TO HAND CARRY AND SELF ADMINISTER AN EPI-PEN, INSULIN OR AN INHALER, THE APPROPRIATE FORMS NEED TO BE COMPLETED BY BOTH PARENT AND PHYSICIAN AND BE ON FILE IN MY OFFICE PRIOR TO THEM
    HAVING THEM ON THEIR PERSON!!**
