Immunizations

In accordance with RSA 200:38 and 141-C, a student is required to be fully immunized prior to the start of the school year.

Physical Exams

Students registering for public school for the first time are required by state law (RSA 200:32) to have a physical examination by a licensed physician. Please forward any results of vision and hearing exams to your school nurse.

The physical exam must take place within one year prior to the first day of school in order to meet entrance requirements. Please schedule appointments early in order to meet the stated deadline.

 A signed and dated physician’s health form must be returned to school as soon as possible.
